Ivy Lodge Retirement Home is the trading name of Ivy Lodge Care Ltd. We are a Residential Care Home providing health and social care for people aged 60 and above. This Privacy Notice tells you about information we collect and hold about you, what we do with it, how we look after it and who we might share it with.
Who is responsible for the data held by Ivy Lodge Retirement home?

What information do we collect and hold about you?
The purpose of collecting and holding certain information about you is to enable us to deliver health and social care to you. The type of information includes;
• Your personal, health and medical information
• Details of your next of kin.
• Details of your Power of Attorney (If applicable)
• Photographs
• Your religion
• Your preferences and interests
What do we do with this information?
The reason we gather and hold this information is to meet our legal requirements and enable us to deliver the appropriate health and social care to you in the manner in which you wish to receive it. Most of this information forms your Care Plan which is a continuously updated document that is held securely on our premises. We may from time to time need to contact your next of kin or Attorney (if applicable) about you and your health or just about projects, updates, fee changes etc at Ivy Lodge.
Who might we share this information with?
In order to deliver the best quality care to you we may need to share some of this information with other healthcare professionals that need it to provide care and support to you including but not limited to the following;
• Your GP
• District Nurses
• Paramedics
• Relevant hospital staff
• Opticians and Chiropodists
We will never transfer your personal information outside the EU without your explicit consent.
For how long do we hold on to your personal information?
We will keep all your information for 8 years after you have left Ivy Lodge unless you exert your right to have it erased. Even then, we may need to keep some or all of your information to meet our insurance obligations or to fulfil other contractual or statutory obligations.
What are my rights under Data Protection legislation?
You have the following rights:
• The right to be informed about the collection and use of your personal data.
• The right of access to that information (by contacting Charles Helfferich)
• The right to rectification should there be any errors
• The right to the erasure of the data
• The right to restrict processing
• The right to data portability
• The right to object; and
• The right not to be subject to automated decision-making including profiling.
